Your Next Steps:
1. Membership Agreement: Upon signing the membership agreement, you will receive your membership card. Our team will immediately begin coordinating refills, tests and treatments, so your healthcare journey remains as seamless as possible.
2. Intake Forms + Scheduling: You will receive an online link to various forms. You will then schedule your first visit with your doctor, and receive any support you need to setup your patient portal and link your membership card.
3. Medical Records Collection: Your health journey begins with a thorough understanding of your medical history. Our dedicated team will collect all available records from the providers shared, as well as screen CommonWell, Athena, and Epic for additional medical histories.
4. First appointment with Your Doctor: As your healthcare co-pilot, we look forward to meeting you and providing you with the expertise, attention, and access you deserve. After we complete your first appointment, you will receive a customized Care Plan that empower you to thrive.
